Medical Requests* Please provide as much information as possible to allow our Clinical Assessment Team to provide a suitable response. Once the submission has been assessed you will receive contact from us with the outcome of the assessment and next steps within 1 working day. For patient safety the medical forms is not available while the surgery is closed. *Admin Requests* Submission is available 24/7 unless the surgery is closed for a Bank Holiday. We aim to respond to these requests within 5 working days. Please DO NOT submit medical requests using this form as it will not be processed. *Repeat Prescriptions* Will be available at your nominated pharmacy within 3 working days, we will be in contact with if there is any delay with your prescription. Acute medications or items not on repeat prescription will take longer to process.
