The Same Day Appointment Request (Medical Query) form is now CLOSED for today. We regret that we are unable to safely process any further urgent medical requests. Medical requests can be submitted again from 8:00am on the next working day. The ADMIN QUERY form will remain open until 6:30pm for routine care requests and administrative queries such as: Letters Medication requests Sick notes Other non-urgent matters .
