Use this form for a new or ongoing health problem, 8:00am to 6:30pm Monday to Friday — a GP reviews every request the same working day. For repeat sick notes, referrals, letters and other admin, use the admin request form instead; those are answered within 5 working days. Tell us on the form if you need an interpreter. For test results, check the NHS App — we’ll contact you if anything needs doing. Can’t use the form? Call 0161 983 0080 and reception will complete it with you.
