Unfortunately, our SAME DAY APPOINTMENT REQUEST (medical query) form is now CLOSED for today. Therefore, we regret that we are unable to safely process any more medical requests. Medical requests can be submitted again from 7:30am the next working day (until we have filled all available appointments). The ADMIN QUERY form will stay open until 6.30pm for any admin queries such as letters, medication requests, sick notes etc.

Online enquiries will be available again on Friday, 02 January at 08:00 AM. (for same day medical requests)
