🕗 Open Mon–Fri, 8am–6:30pm Please note that forms submitted between 4pm and 6:30pm will not be looked at until the next working day - do not submit an urgent request between these times
⏰ Urgent, same-day requests (incl. home visits) must be submitted before 11am
🕚 After 11am, urgent needs may be redirected to NHS 111 or 999 in an emergency
⚠️ If your condition worsens, do not wait – call 111 or 999 in an emergency.
Do not use the admin/routine care form for a medical request as this will not ensure you receive medical prompt medical treatment.
