You can use our online form for any type of routine request such as non-urgent appointments, prescriptions, test results, or general admin queries. 🕗 Service Hours: Open Monday–Friday, 8:00am–6:30pm (excluding bank holidays). ⚠️ Important: If your condition gets worse after completing the form, DO NOT WAIT FOR A REPLY — call 111 or 999 immediately. Please use the medical request form for any non-urgent health related concerns. Admin/routine forms are not constantly monitored and may delay your care if you complete this form with a medical problem. Once we’ve received your request, a member of our Care Navigation Team will review it. We will let you know within 1 working day how we will handle your request (this may include arranging an appointment on another day or signposting to another appropriate service). If your symptoms worsen, please call the practice on 0161 226 0606 during opening hours, or dial 999 for life threatening emergencies. Please be aware that urgent on the day appointments are not unlimited. ☎️ Need help completing the form? Our Care Navigation Team will be happy to assist you. Please pop in to the practice or call 0161 226 0606 so that this can be arranged for you. ⏰ For urgent, same-day needs: 📱You can book a same day appointment via the NHS App (appointments are released at 8am for urgent issues and at 12pm for routine issues, these appointments are in 6 days time). All appointments booked via the NHS app are reviewed by our Care Navigation team who may need to contact you for further information, so please keep your phone handy after booking. 📞Telephone the practice on 0161 226 0606 from 8am. We ask that you keep our phone lines free for those who need it most, so if you can use the NHS App or complete the online form, please do so. 🚶‍♂️Walk in and book via our front desk. Please note that urgent on the day appointments are not unlimited. Once all of our urgent appointments have been taken, you may be asked to contact NHS 111 or, in an emergency, call 999. 🏠For home visits: These must be telephoned through to the practice on 0161 226 0606 by 11:30am.
