Online forms are intended for routine medical and administration requests only. Requests will be reviewed by our triage team whilst the practice is open (this does not include outside of practice opening times, weekends, bank holidays or during protected practice training sessions). If your request is triaged as clinically urgent a member of our healthcare team will respond to you the same day. When the practice is closed please contact an alternative healthcare provider such as NHS111 if you have run out of medication or require general medical advice.
