This online form is the quickest and simplest way for majority of our patients to access our clinical team for assistance. Your form will be triaged by a clinician between 8am and 6.30pm • Patients with urgent matters will be contacted the same day where appointment availability allows. • Patients with non-urgent matters will be contacted within 48 hours to arrange appropriate follow up. Provide as much information as possible to enable us to triage you request appropriately and respond in a timely manner. Do not use this form to request repeat medication or to request a blood test appointment, instead continue to use patient access services or the NHS App.
