Please fill in this online form for all appointment requests (excluding blood tests, smears, vaccinations, travel clinics - for which you can telephone reception directly). When submitting please ensure you complete the form with the exact details that appear on your medical record otherwise your form will not be matched to your record. Please do not use any abbreviations or shortened names. Once submitted your form will be triaged by a clinician ** between 8am and 6.30pm ** Patients with urgent matters may be contacted the same day, please keep your mobile phone close by as you may receive instructions, requests for further information or the triage GP may request that you come in to the Practice or be available for a telephone appointment. Please note if you are unable to accept the urgent appointment offered you may not be offered an alternative as an urgent due to limited capacity • Provide as much information as possible to enable us to triage you request appropriately and respond in a timely manner. *PATIENT NOTICE: If you are abroad and need urgent medical advice or treatment you must seek medical attention in the country you are in. Similarly if you are aboard and have a non-urgent request please wait to submit this when you are back in the UK as we will often respond the same day.
