Your request will be read and responded by a member of the team on the same working day, provided its submitted during surgery opening hours of Monday-Friday 8 am - 6.30 pm. We may return your request if it is not appropriate or we do not have enough information, so please do ensure you have filled it out with as much information as possible. If you have an URGENT medical concern please call us on 0121 360 8560. If a pharmacy has submitted a prescription request on your behalf, please note this will be rejected. As our patient, please kindly submit a request yourself using the triage link.
