This request will be triaged by a member of our team on the same day that your request is submitted. (operating hours 08.00-18.30 Monday to Friday, closed on weekends and bank holidays). For any medical requests, you may be requested to provide further information to assist our team with triaging your concerns. For any medical queries regarding skin concerns, eyes, swelling, infections or rashes, we would request appropriate photographs to be sent in along with the query. Please do not include any photography of an explicit nature. Failure to include this important information may lead to a delay to triaging and a further request being sent back to you. For any queries regarding recent discharges from hospital, there is a process in place for the surgery to receive and action the discharge paperwork and initiate appropriate follow-up actions. This process is dependent on secondary care processing your information in a timely manner. If you have been recently discharged from a hospital please follow the instructions for the medications as listed on your discharge summary, until this is formally reviewed. If you have an urgent query regarding your discharge paperwork, please contact our dedicated team on 01214761135 who will support you with your query. Repeat medications can now be requested through the NHS APP. Please do not submit an online request via this method. If you do not have access to the NHS APP please contact the prescription ordering team on 01217261106. We are not able to "chase" referrals on your behalf. If you have a concern regarding the length of time you have been waiting to receive an appointment after being referred, please make contact with that service directly.
