All 'I have health problem' request forms will be reviewed and assessed by a doctor or advanced practitioner on the day during our opening hours 08.30 - 18.30 (excluding weekends and bank holidays). The request will be allocated to the most appropriate team member, please note: any messages received after 18:00 hours may not be read until the following morning. If you require test results, please fill out an 'I have an admin or routine care request' form or call the results line on 01373 867986 between 09:30 – 13:00 and 14:00 – 16:00, Monday to Friday.
