The Patient Triage form is now closed, The Triage Form is available between 7:30am - 6pm Monday – Friday. PLEASE DO NOT EMAIL THE HEART OF BATH TO BOOK APPOINTMENTS. All Nurse appointments are prebookable and can be requested via the Admin form and requests for urgent or routine GP or clinical appointments need to be submitted via the Health form, these are triaged and GP/Clinical appointments are aimed to be offered for same day appointments so please contact us on the day you are able to come to the surgery.
