Please complete our online form below for any medical or admin queries. Once completed, a member of staff will aim to deal with your request as soon as possible. For medical queries, we aim to respond directly to your request either by email, text or phone call within 48 hours and admin queries can sometimes take longer. If a doctor decides that a medical request is urgent then it will be assessed and completed on the day. You will not receive a confirmation email or text, so please ensure that you make a note of your request ID for reference. Please avoid calling the practice to follow up on your request as this puts unnecessary pressure on our admin team. You can find the wait times of the different request on our online services page.
