This short online form is the quickest and simplest way to access our clinical team. Our phone lines remain open for anyone with accessibility needs who might find it difficult completing this form. For those without accessibility needs, our reception team may direct callers to complete this online form. All forms will be assessed by a clinician or administrator, depending on the nature of your request, we aim to review your request and respond back by the end of the day if received before 5.30pm, otherwise we will get back to you by the next working day excluding Bank holidays. For all admin and non-urgent requests please allow up to 2 working days, if after this time you have not heard back, please contact reception. Please be aware that you may now receive a response from the practice via your email address or NHS App, so it's important to ensure you have your notifications switched on. If you do not have the app downloaded, the practice will continue to send you any messages via Email/SMS. Furthermore, if you do not read your message in the NHS App within 60minutes, we will send it again via SMS to ensure it is received.
