Our health problem form is available Monday-Friday (excluding bank holidays) between 0800-1700 (if capacity allows). If you have a medical enquiry outside of these hours please either contact 111, call reception, or submit your enquiry the next working day from 08:00. Our admin form is available Monday-Friday (excluding bank holidays) between 0800- 1830. Admin requests go through to reception and will be processed in the next 2 working days. Please ensure that you use the correct form as medical requests will not be actioned if received as an admin request. Prescriptions can also be requested 24/7 via the NHS app.
