Please use our health problem form for all GP appointment requests. Our health problems form is available Monday-Friday between 08:00-1700 (unless clinically overwhelmed). If you have a medical enquiry outside of these hours, please either call reception, contact 111, or submit your enquiry the next working day from 0800. Please ensure you use the correct form. If you have 2 or more health problems, please use a separate form for each problem. Please also submit one form for each person i.e. if you are submitting a health problem for different family members, please submit on separate forms. We would really appreciate if you could put clearly your availability for an appointment if this is required. Urgent health problem requests will be responded to the same working day. If we are unable to reach you on two occasions you will be asked to resubmit your request. Our admin form including routine review appointment request is available Monday-Friday (excluding bank holidays) between 0800- 1830. Admin requests go through to reception and will be processed in the next 2 working days. Please ensure that you use the correct form as medical requests will not be actioned if received as an admin request. Prescriptions can also be requested 24/7 via the NHS app.
