Our website is open for medical requests from Monday to Friday from 8am-3pm only, these will be sent to a GP for triage and responded to according to urgency. Administrative and routine requests can be submitted at anytime using our admin form. Please note routine care requests are specific to certain long term condition reviews, screening and routine vaccinations. These are not triaged by a GP, are subject to eligibility criteria (for example, age, medical history, or timing since your last check) and must not be used for new or changing symptoms. If you have a new or developing medical problem, you must submit this Medical Request form within our opening hours so that a GP can triage your care appropriately.

If you need more urgent help, call your GP practice. If it's closed, visit NHS 111 online or call 111. In an emergency call 999
