Each submission must be for one person and one issue.
Please give as much information as possible.
Your request will be triaged within one working day - this may be a call back, an appointment, message or signposting to community services according to the triage urgency determined by the duty doctor based on the information you provide.
If you have an urgent matter, please call reception for further advice.
Prescription requests will go to the prescribing team and may take up to 5 working days.
If you need more urgent help, call your GP practice. If it's closed, visit NHS 111 online or call 111. In an emergency call 999
