***This service is only for patients registered with Ordnance Unity Centre for Health***. This short form is quickest and simplest way to ask the surgery for medical help. Please include as much information as possible and only complete one form per medical issue. Medical requests will usually be processed on the same day and the medical team will prioritise requests based on urgency. Repeat prescriptions will be processed within 2 full working days and other admin requests may take up to 28 days. Please be patient with our team as we also adapt to the new system. Working days do not include weekend and bank holidays. DO NOT SUBMIT MULTIPLE REQUESTS FOR THE SAME ISSUE!
