Online requests can be submitted between the hours of 8am and 8pm on weekdays (excluding bank holidays), and between the hours of 8am and 1pm on weekends. Please note, the practice is open from Monday to Friday, 8am to 6.30pm. Requests will be processed only when the practice is open and reviewed within 2 working days of receipt.
Because requests are not reviewed immediately, please do not use this form for urgent matters, or for an acutely unwell child or young person under the age of 16.
