This triage form must not be used to request repeat medications – please use the NHS App, or email gower.prescriptions@nhs.net instead. Please note that if the form is submitted after 5 PM, it may not be seen by a clinician until following working day, and we can only provide advice to patients who are currently in the United Kingdom.
