This service is for 1 problem per message and will be read by a member of our team Mon- Fri between 8am and 18.30pm. Please be aware we are not able to process requests on the incorrect form. Incorrect form use, might delay you receiving the help you need and you may be asked to re submit it again using the correct form. For medical queries: your request will be reviewed on the same day it is received (during normal working hours). A member of our clinical team will contact you within 2 working days, depending on clinical urgency.
