All requests are to be submitted online except for URGENT ON THE DAY requests. If you feel your request is URGENT AND MUST BE DEALT WITH ON THE DAY, PLEASE PHONE RECEPTION. If your concern could be life threatening, please call 999. Please only submit one problem per form. If you submit more than one problem per form or not enough information on the form, our admin team will respond to say the form has been rejected. Please use this service responsibly. Sending multiple requests for minor issues can overwhelm the system and slow down care for others. You will receive a response to your request within 2 working days. Please bear in mind, IF YOUR REQUEST IS DEEMED URGENT BY THE DUTY DOCTOR, YOU MAY BE ASKED TO ATTEND THE SURGERY IN PERSON AT SHORT NOTICE. The GP will only try to call you once so please ensure you have your phone with you.
