Following triage, we will contact you with the next steps. This may include an appointment at the practice or signposting to alternative services, such as Pharmacy First or other support within our Primary Care Network (PCN). If you are unable to complete the online form due to accessibility needs, please contact the practice. A member of our team will assist you or provide a paper form.
