If you feel you may need an appointment on the same day, please ensure you complete and submit the triage form by 10:00 am. You will receive a response within 48 hours of your triage request being submitted. Please submit a separate form for each patient. If your request is not appropriate for the GP Practice, you will receive an SMS notification with details of other suitable services.
