The URGENT medical online triage form is now closed as we have reached the safe daily capacity limit for urgent issues. The routine care and admin form is open BUT it is NOT suitable for URGENT medical issues. If you have an URGENT medical issue, please TELEPHONE the surgery and your request will be passed to the on-call doctor. Please ONLY use the admin/routine medical request form if you have a NON-URGENT query about an appointment, administrative or medication matter, or a request that doesn't need immediate attention. Please note that, once submitted, we cannot guarantee an immediate response but we do aim to review and take necessary action within ONE WORKING DAY.
