The Health Problem online triage form is now closed as we have reached safe capacity. If you have an urgent medical issue that needs to be dealt with today, please call the surgery or NHS 111. The admin or routine care request online form is available during surgery working hours for non-urgent appointment requests, medication queries and admin requests. Your request will be reviewed by our clinical or administrative team. Submissions are reviewed during our normal working hours, and we will provide a response once your request has been assessed. Response times can differ depending on the nature of your enquiry and overall demand.
