IMPORTANT FOR PATIENT SAFETY : Forms submitted after 6:30pm will not be reviewed until the next working day ONLY ONE MEDICAL ISSUE PER FORM. If more than one medical problem is listed the GP will triage the most urgent problem. You will be contacted to resubmit further forms for the other problem(s). One problem, one consultation.
