To help us support you quickly and effectively: • Complete all sections of the form. • Provide as much detail as possible (e.g., Are symptoms constant or intermittent? Do you feel unwell or feverish? Are there other symptoms? Is this a recurring issue?). • Let us know if someone has already been helping you with this problem so we can maintain continuity of care.
