If you are not available that day, please submit on a day you are available to attend an appointment. Please note Monday and Tuesday can be particularly busy.

Common issues to avoid:
• Using the wrong name or date of birth, especially for children.
• Using nicknames or abbreviations (e.g., Mickey instead of Michael), middle names as first names, or married names not yet updated in our system.
• For date of birth, some months only require a single digit (e.g., July = 7, not 07).

To help us support you quickly and effectively:
• Complete all sections of the form.
• Provide as much detail as possible (e.g., Are symptoms constant or intermittent? Do you feel unwell or feverish? Are there other symptoms? Is this a recurring issue?).
• Let us know if someone has already been helping you with this problem so we can maintain continuity of care.
