To request an appointment at one of our surgeries please submit an online request form. This will be reviewed within 48 hrs and signposted to the most appropriate service. This may include an appointment at the surgery, attending your local pharmacy, or one of our other services - first contact physio, mental health service, or social prescriber.
If your total triage form is submitted after 5pm you may not receive a response until the following working day.
