Our medical request forms are open from 8:00pm to 6:30pm, Monday to Friday. Please do not use administrative forms to submit a medical query - we won't review them. Our administrative forms are available 24 hours, 7 days a week. Please allow 72 hours for us to get back to you regarding your administrative query.
