This service is for one problem per form. Submissions are reviewed by a member of our team Monday to Friday between 8:00am and 6:30pm (excluding bank holidays). For medical queries, you will be contacted on the same day you submit the form, either by text message, telephone, or you may be asked to attend in person—please ensure you are available. Please do not use this form for administrative queries if your concern is medical.
