What would you like help with? “I have a health problem” This option is for acute health problems, that you feel needs to be reviewed on the day, and we will respond to you on the same day with an outcome. Please note the outcome may be advice from the clinical team, self-help advice, referral to a local pharmacy, or, if clinically necessary, an appointment will be booked for you at the practice. The triage team will assess the urgency of your request, and if required, an appointment will be made for you on the day or within 14 days of your request. It is very important to state your availability for all of these timeframes on your triage form. If you do not state times that you are not available, and an appointment is booked for a time that you cannot attend, you may be asked to complete another triage form. We will try to accommodate your request, but please be aware this may not always be possible. “I have an admin or routine care request” This option is for fit (sick) notes, repeat prescription requests, reviews of chronic health conditions and medications, screening, and vaccinations. When medical triage is closed, please refer to the below options: 1. Ring the surgery on 01621 876433 if your request is urgent. The surgery is open Monday to Friday from 08.00 – 18.30 2. Contact us via the “I have an admin or routine care request” option for any routine matters. You will receive a response within 3 days. 3. Contact us via SystmOnline for routine matters. You will receive a response within 3 days. 4. Visit your local pharmacy for advice.
