If you are requesting a routine appointment, please state whether you would like this to be a Face to Face appointment or a telephone call. If you request is regarding a sick note, please be aware that this is only for extending current sick notes. If you need a new sick note, you will need to book a routine appointment with a GP. Please note sick notes can be backdated and will not be seen as clinically urgent.
